What is a Geriatric Care Manager?

Think of your Geriatric Care Manager as a “professional” son or daughter, an advocate who works with seniors and families to assure that needed senior services are delivered in a quality and timely manner. A care manager will set up an individualized care plan, arrange for the assistance required, then continually work with seniors and families to ensure that a senior’s needs are being met.

 

Maintaining an Independent Lifestyle

The Council on Aging’s Care Managers are on call to provide seniors and their families with a flexible array of cost-effective services to help seniors remain at home. Services include everything from assistance with shopping to monitoring of medications and home health care. We begin the process via phone, then pay a personal visit to the senior to assess which services are most appropriate to meet needs.

 

The Care Manager can professionally screen, hire and monitor in-home services including:

  • Companion services (including escorts for physician appointments and shopping)
  • Homemaking
  • Assistance with bathing and personal care
  • Medication management by a registered nurse
  • Respite care to relieve a stressed family caregiver
  • Assistance with bill paying and insurance/medical claims processing
  • Planning of long-term care options
  • Daily at-home meal delivery through Meals on Wheels
  • Transportation via Community Coach

 

 

For those seniors who would benefit from consistent health care, supervision and socialization during the day, the geriatric care manager can arrange for adult day care. (Return to the home page and choose the adult day care section for a description of this service.).

 

A Geriatric Care Manager also acts as an advocate for seniors with physicians, lawyers and other key advisors. Most importantly, our Geriatric Care Manager provides regular updates to long distance family members, which brings peace of mind to out-of-state children or grandchildren.

 

In-town caregivers can benefit from monthly support groups meetings, which are free of charge and lead by a Master’s level Geriatric Care Manager.

What are the costs associated with Geriatric Care Management service?

Since Medicare does not pay for Geriatric Care Management, there are several options for payment.  Financial assistance for assessment and fees is available for qualifying seniors through funds donated to the Council on Aging.  Some seniors are asked to make a donation toward the costs.  Some seniors make a co-payment with government funding paying the balance.  And, some seniors pay a fee covering the entire cost of the service.  The payment option is determined through the interview and intake process.
